Omnitida BioActive is a prescription medicine containing tirzepatide, a dual GIP and GLP-1 receptor agonist, for once-weekly subcutaneous injection. It is indicated as an adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes, and for chronic weight management in adults with obesity, or with overweight and a weight-related comorbidity.

The range includes ready-to-use pre-filled pens with four weekly doses in 2.5, 5, 10 and 15 mg strengths, and 2 mL vials for administration by medical staff in 2.5, 7.5 and 15 mg per 0.5 mL strengths.

Active ingredient: Tirzepatide

Omnitida contains tirzepatide, a 39-amino-acid modified peptide with a C20 fatty diacid moiety. Binding to albumin prolongs the half-life to about 5 days, allowing once-weekly dosing.

It acts to:

  • Enhance first- and second-phase insulin secretion in a glucose-dependent manner
  • Reduce glucagon levels in a glucose-dependent manner
  • Delay gastric emptying

Flexible dosing regimen

Stepwise titration to improve tolerability:

  • Start: 2.5 mg once weekly for 4 weeks (initiation, not for glycaemic control)
  • Titration: +2.5 mg after at least 4 weeks on the current dose
  • Maintenance dose: 5, 10 or 15 mg once weekly, as prescribed
  • Maximum dose: 15 mg once weekly
  • Subcutaneous: abdomen, thigh or upper arm; any time of day, with or without meals

Dual-receptor action

Tirzepatide selectively binds to and activates both GIP and GLP-1 receptors — the targets of the native incretin hormones.

Indicated for:

  • Glycaemic control in adults with type 2 diabetes, as an adjunct to diet and exercise
  • Chronic weight management in adults with BMI ≥ 30 kg/m², or ≥ 27 kg/m² with a weight-related comorbidity

The most common side effects are nausea, diarrhoea, decreased appetite, vomiting, constipation, dyspepsia and abdominal pain. Warnings include pancreatitis, hypoglycaemia with insulin or sulfonylureas, hypersensitivity reactions, renal impairment with severe GI reactions, diabetic retinopathy and cholelithiasis. What does the pen kit include?

One pre-filled pen with 4 once-weekly doses, 4 needles (one per dose), alcohol wipes and the package leaflet. Vials are supplied with the package leaflet only; syringes are not included.

How does Omnitida BioActive work?

Tirzepatide activates both GIP and GLP-1 receptors. It enhances first- and second-phase insulin secretion and reduces glucagon levels, both in a glucose-dependent manner, and delays gastric emptying.

Who should not use Omnitida BioActive?

It is contraindicated in patients with hypersensitivity to tirzepatide or any component, and with a personal or family history of medullary thyroid carcinoma or MEN 2. It has not been studied in patients with a history of pancreatitis, is not indicated in type 1 diabetes and is not established below 18 years of age.

How is Omnitida BioActive administered?

Once weekly by subcutaneous injection in the abdomen, thigh or upper arm, at any time of day, with or without meals. Rotate injection sites with each dose. With insulin, give separate injections and never mix them.

What if a dose is missed?

Give it as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip it and give the next dose on the usual day. The weekly day can be changed if at least 3 days (72 hours) separate two doses.

How should Omnitida BioActive be stored?

In a refrigerator at +2 °C to +8 °C, in the original carton to protect from light. Do not freeze and do not use a product that has been frozen. Keep out of the reach and sight of children.

How is the dose adjusted over time?

Treatment starts at 2.5 mg and is increased in 2.5 mg steps after at least 4 weeks on each dose. The maintenance dose (5, 10 or 15 mg) is chosen by the prescribing physician based on response and tolerability; if a dose is not tolerated, a lower maintenance dose may be considered.
